Care Management Nurse Supervisor:

Family Summary/Mission
-Provide comprehensive healthcare management to facilitate delivery of appropriate quality healthcare, promote cost effective outcomes and improve program/operational efficiency involving clinical issues.
-Responsible for management of medical services staff.
-Works closely with managers (re: Case/Disease Management)
-Inpatient Care Coordination/Discharge Planning/Case Management/Disease management/Precertification and monitor daily performance.
-- Implements clinical policies & procedures in accordance with NCQA, URAC, state and federal standards and mandates.
-Serve as a content model expert and mentor to the team regarding practice standards, quality of interventions, problem resolution and critical thinking (*)
-- May collaborate/deliver inter and intra-departmental training sessions (*)
-- Select staff using clearly defined requirements in terms of education, experience, technical and performance skills
-¿ Sedentary work involving periods of sitting, talking, hearing, work requires visual acuity to perform close inspection of hand written and computer generated documents as well as a PC monitor. Typical office working environment.

Educational preferences:

¿ Requires an current unrestrictive state licensure RN depending on state requirements, BSN desirable.
¿ Specific to ActiveHealth Mgmt: When the supervisor has directly supervised the CM process for at least three (3) years with ActiveHealth Management, a certification in case management from the URAC-approved list of certifications is required.

Additional Information:
¿ Computer literacy and experience familiarity with Excel, MS Word, and web-based applications desired.
